Eye exam before starting the plaquenil

Chloroquine is now uncommonly used in favor of its derivative hydroxychloroquine. In the United States, hydroxychloroquine is most often used for its anti-inflammatory effects in rheumatology and dermatology.

A 26-year-old black female presented in office for her annual comprehensive eye exam. Her medical history was significant for Plaquenil therapy spanning 12 years with a daily dose of 200mg twice daily. She presented with a best-corrected visual acuity of 20/20 OD and 20/20 OS. All preliminary testing revealed normal findings. Both my rheumatologist and opthamologist recommended exams every 6mo for the first year I was on plaquenil. Now I just go yearly to have my eyes checked. Also, my rhuemy said any time he makes an adjustment to my plaquenil that I'll need an eye exam within 6mo just to make sure nothing has changed. Good luck! As a precaution, patients treated with Plaquenil are advised to get a baseline eye exam prior to starting the drug and then annually thereafter. This information provided by Neil I. Stahl, MD & Tongalp H. Tezel, MD was first printed in the The Moisture Seeker, SSF's patient newsletter for members.

While early toxicity may be asymtomatic, patients with more advanced stage of toxicity may complain of color vision changes or paracentral scotomas. Its toxic effects on the retina are seen in the macula.

Most rheumatologists recommend patients undergo a baseline eye examination prior to starting Plaquenil and be re-examined in the future depending on their risk for developing the condition. Risk factors include advanced age and having pre-existing retinal disease.

Remember, any patient who is at a high risk for Plaquenil toxicity should be examined on an annual basis following the baseline exam. The goal of monitoring/screening is to identify early macular damage prior to irreversible vision loss or even before visible signs of bull’s eye maculopathy. The rheumy suggested trying the Brand name, instead of generic. That did the trick. No problems whatsoever with that. It takes weeks to months before you will feel the full effect of the drug. Also - regular eye exams are important while taking Plaquenil - either yearly or twice yearly. My rheumy required a baseline eye exam before starting me. Great that you're getting a baseline eye exam before you start taking it so that the doctors have something to compare to at subsequent visits. You should get a Plaquenil eye exam yearly on this medication. For many, Plaquenil has helped symptoms a lot.
